Welcome back to our weekly edition of Friday Field Notes.

This week inside our Doctor of Functional Medicine program, we explore a topic that affects nearly every system in the body, but is often assessed through an incomplete lens: Thyroid Testing. 🦋

The thyroid may be small, but its influence is enormous.

From metabolism and energy production to mood, cognition, cardiovascular health, temperature regulation, and hormone balance, thyroid hormones help orchestrate countless physiological processes throughout the body.

Yet for many individuals experiencing symptoms of thyroid dysfunction, standard testing often stops with a single marker: TSH.

While valuable, TSH represents only one piece of a much larger story.

The Rockwell approach recognizes that thyroid health is not simply about hormone production—it's about hormone conversion, transport, cellular utilization, immune function, and the many factors that influence those processes behind the scenes.

🦋 Beyond TSH

TSH (Thyroid Stimulating Hormone) is often considered the gold standard of thyroid testing, but it is ultimately a signaling hormone produced by the pituitary—not a thyroid hormone itself.

A more complete assessment may include:

  • Free T4 — Measures the unbound form of thyroxine available for conversion.
  • Free T3 — Measures the biologically active thyroid hormone used by cells.
  • Thyroid Patterns — Evaluating how these markers work together can provide a more complete picture of thyroid physiology.
  • Functional Perspective — Symptoms may sometimes appear long before lab values reach conventional disease thresholds. 

Looking beyond a single marker allows practitioners to better understand what may be happening throughout the entire thyroid cascade.

🔄 Reverse T3 & Thyroid Conversion

To become biologically active, T4 must be converted into T3 through specialized enzymes found throughout the body.

Sometimes, however, T4 is converted into Reverse T3 (rT3)—an inactive metabolite.

Elevated Reverse T3 may be associated with:

  • Chronic stress
  • Inflammation
  • Nutrient deficiencies
  • Liver or kidney dysfunction
  • Cellular energetic imbalances ⚡

Assessing thyroid conversion patterns can provide valuable insight into how efficiently thyroid hormones are being utilized at the cellular level.

🧪 Thyroid Antibodies & Binding Proteins

Many thyroid disorders have an autoimmune component that can begin years before changes appear on standard thyroid panels.

Additional testing may include:

  • Thyroid Peroxidase Antibodies (TPOAb)
  • Thyroglobulin Antibodies (TgAb)
  • TSH Receptor Antibodies (TRAb)
  • Thyroxine-Binding Globulin (TBG)
  • Albumin
  • Transthyretin

These markers help assess immune activity, hormone transport, and other factors that can influence both thyroid function and the interpretation of thyroid laboratory findings.

🤍 Thyroid assessment is not simply about asking whether the thyroid is functioning—it's about understanding how the entire system is working together.
